Client Consent Forms

In order to uphold social distancing measures, I have chosen to go digital with all consent forms and questionnaires. This has been put in place to lessen the amount of time you would have to spend in waiting areas. You are more than welcome to complete these out before your appointment request is accepted or before you book your appointment. However, all required forms pertaining to your treatment must be completed at least 24 hours before your service.
